Abstract :ÖZET Amaç: Bu çalışmada kliniğimizde kolesistektomi yapılan olgular içerisinde raporlanmış Safra Kesesi Poliplerinin (SKP) klinik ve histopatolojik olarak değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Kliniğimizde 2007-2016 yılları arasında yapılmış olan 5212 kolesistektomi vakası retrospektif olarak incelendi. Hastalar preoperatif ultrasonografi ve postoperatif patoloji sonuçlarına göre değerlendirildi. 241 hasta preoperatif ultrasonografilerinde multiple veya tek sayıda polipoid lezyon olması nedeni ile opere edilmişti. Bulgular: Hastaların ortalama yaşı 44,65 yıl olup en genç hasta 21 en yaşlı hasta 84 yaşındaydı.145 hasta kadın, 96 hasta erkek idi. 157 hastanın semptomatik şikayetleri var iken, 84 hastada herhangi bir semptom yoktu. Kolesistektomi sonrası patolojik incelemelerde 58 ve 57 yaşlarında olan 2 kadın hastada safra kesesi karsinomu saptandı. (% 0,83) 66 yaşındaki bir diğer kadın hastada da düşük dereceli displazi mevcuttu. Opere edilen diğer hastalarda malign veya premalign lezyon saptanmadı. En çok rastlanan patolojik sonuç 149 vaka ile kolesterol polibi idi. Bunun dışında, başka bir patoloji olmaksızın olan kolelitiazis, koleterolozis, kronik kolesistit, fibroepitelyal polip, hamartamoz polip, inflamatuar polip ve adenomyoma görülmüştür. Sonuç: Safra kesesi kanserleri yaşla birlikte artmakta ve kadınlarda daha sık görülmektedir. Bizim çalışmamızda görülmüş olan 2 kanser vakası da 50 yaşın üstünde olan kadın hastalardır. Safra kesesinin polipoid lezyonları değerlendirilirken hastanın semptomatik olup olmaması, yaşı, cinsiyeti, ultrasonografik olarak polibin boyutu, çap artışı, çok sayıda olup olmaması göz önünde bulundurulmalıdır. Bu sonuçlar sağlık eğitimi, sağlık politikası ve gelecekteki araştırmalar için değerli bir kaynak olabilir. Daha ileri özelliklerini tanımlamak için daha geniş çapta çalışmalara ihtiyaç vardır. Materyals and Methods: 5212 cholecystectomy cases performed between 2007-2016 in our clinic were retrospectively reviewed. Patients were evaluated according to preoperative ultrasonography and postoperative pathology results. 241 patients were operated on preoperative ultrasonography for multiple or single polypoid lesions. Findings: The mean age of the patients was 44,65 years. The youngest patient was the 21 years old. The oldest patient was 84 years old. While 157 patients had symptomatic complaints, 84 patients had no symptoms. Pathologic examinations after cholecystectomy revealed bile duct carcinoma in 2 female patients aged 58 and 57 years. (0.83%) Another female patient aged 66 years had low-grade dysplasia. Malignant or premalign lesions were not detected in the other patients. The most common pathologic result was cholesterol polyp with 149 cases. Apart from this, other pathologies without cholelithiasis, cholesterolosis, chronic cholecystitis, fibroepithelial polyp, hamartamoz polyp, inflammatory polyps and adenomyoma are seen. Conclusion: Gallbladder cancers increase with age and are more common in women. The two cancer cases that have been seen in our study are female patients over 50 years of age. When polypoid lesions of the bile duct are evaluated, consideration should be given to whether the patient is symptomatic, age, sex, ultrasonographic polyp size, diameter enlargement, and large numbers. These outcomes may be a valuable resource for health education, health policy and future research. There is a need for larger workloads to describe more advanced features.
