In this case report, we aimed to present the ultrasound-guided infraclavicular block technique due to upper extremity surgery patient who have bilateral bullous lung disease. 50-year-old male patient who had severe bilateral bullous lung and with a history of spontaneous pneumothorax was admitted to our hospital because of the right olecranon fracture. Ultrasound-guided nerve block and simultaneous nerve stimulation was planned to perform the right infraclavicular block. Surgery was performed without any complications and patient was transferred to ward. Postoperative analgesic was not necessary until the eight hours after surgery. As a result, regional anesthesia could safe technique compared to general anesthesia particularly patients who have severe pulmonary comorbidities. We also think that the combination of ultrasound and nerve stimulation are minimized the complications such as pneumothorax and intraneural injection during peripheral nerve block interventions.
