Abstract :Venöz ülserasyonlar yetişkin populasyonun yaklaşık %1’ini etkileyen önemli hastalıklardandır. Yüzeyel ve derin venöz yetmezlik, venöz akım tıkanıklığı ve baldır kas pompasının yetmezliği ile ilişkili olan venöz hipertansiyonun venöz ülserlerin gelişiminde altta yatan neden olduğu düşünülmektedir. Doku hipoksisi, inflamasyon ve lenfatik disfonksiyonun bir arada olması venöz ülserasyona yol açabilir. Ancak, venöz ülser gelişmesinde altta yatan biyokimyasal mekanizma tam olarak bilinmemektedir. Venöz hipertansiyonun interstisyel mesafeye sıvı ve moleküllerin kaçışına neden olarak doku iskemisine yol açtığı hipoteze edilmiştir. Bu kısmi iskemik durum; serbest radikallerin ve venöz ülserasyonun oluşmasında muhtemelen esas nedendir. Dereceli kompresyon tedavisi uygulanması tedavide dayanak noktasıdır ve ülsere yaraların iyileşmesini çabuklaştırır. Pentoksifilin ve purifiye flavonoid fraksiyonu venöz ülserlerin tedavisinde etkili bulunmuştur. Ülsere defektlerin cilt greftleri veya biyomühendislik ürünü yapay derilerle kapatılması ancak geçici bir iyileşme sağlar. Üstelik venöz hipertansiyonun mevcudiyetinin devam etmesi nedeniyle nüks oranları da oldukça yüksektir. Kompresyon ve tıbbi tedaviye ek olarak venöz hipertansiyonu ve nüksleri azaltmak için uygun olgularda cerrahi girişimler de yapılmalıdır. Tüm bu tedavi yöntemlerine rağmen nüks olabilir. Bu yüzden venöz hipertansiyonla ilişkili biyokimyasal olayları aydınlatmak için ileri çalışmalar gereklidir. Anahtar Kelimeler: Venöz ülser; Venöz yetmezlik; Oksidatif stres. ABSTRACT Venous ulceration is an important disorder affecting about 1% of the adult people. Venous hypertension is thought to be the underlying cause of the development of venous ulcers, related with superficial and deep venous insufficiency, venous flow obstruction and failure of the calf muscle pump. The combination of tissue hypoxia, inflammation and lymphatic dysfunction may lead to venous ulceration. However, the underlying biochemical mechanism of venous ulcer development is not known exactly. It has been hypothesized that venous hypertension produced an egress of fluid and molecules into the interstitium leading to tissue ischemia. This partially ischemic condition may be the basis for generation of free radicals and venous ulceration. The application of gradually compression therapy results in accelerated healing of ulcerous wounds and has been the mainstay of the management. Pentoxifylline and purified micronized flavonoid fraction have been found to be effective for treatment of venous ulcers. Coverage of the ulcerous defects with skin grafts or bioengineering artificial leather provides temporary healing. Moreover, recurrence rates are very high due to continued presence of the venous hypertension. In addition to the compression and medical therapy, surgical interventions should be performed to decrease the venous hypertension and recurrence rate in appropriate cases. Despite all of these treatment modalities recurrence may be inevitable. Thus, further studies are required to elucidate the biochemical events associated with venous hypertension.
